Borst reductie en borst lift

Wanneer u last heeft van te zware en/of hangende borsten dan kan dit gepaard gaan met lichamelijk ongemak, zoals nek- en rugklachten, overmatig zweten en knellende bh-bandjes. Ook psychische klachten kunnen voortkomen uit schaamte voor de omvang en/of vorm van uw borsten. Wij kunnen u hiermee helpen door een borst verkleinende operatie of borst lift uit te voeren. Beide zijn chirurgische ingrepen die qua behandeling hetzelfde zijn, alleen bij een borstreductie wordt er ook weefsel weggehaald. Ook is het bij een borstlift mogelijk uw borst te vergroten met een prothese. De operatie vindt plaats onder algehele anesthesie en duurt ongeveer 1 ½ uur – 2 uur. De ingreep kan als een dagbehandeling plaatsvinden.

De plastisch chirurg houdt zoveel mogelijk rekening met uw wensen en tijdens een persoonlijk consult zullen de mogelijkheden uitgebreid met u besproken worden. Zoals bij alle operaties is het belangrijk dat u een goede gezondheid en gewicht heeft.  Ons doel is dat u een realistisch verwachtingspatroon heeft van de mogelijkheden, risico’s en nazorg. Tijdens dit gesprek luisteren we naar uw wensen en bespreken we de mogelijkheden. Geen enkele vrouw is hetzelfde en daarom is het belangrijk dat u samen met de arts kijkt naar uw figuur en de juiste verhoudingen. In samenspraak met de chirurg maakt u een keuze die voldoet aan het door u gewenste resultaat.

Heavy and/or hanging breasts can give back, shoulder and/or neck pain. Bra straps can cut and rub into the skin of the shoulders and sweat under the breasts can cause irritations and fungus infections. An abnormal shape can also be experienced as repulsive or shameful. In most women a small asymmetry can be observed whereby one breast is slightly larger, and/or is hanging lower and/or has a slightly different shape. When the asymmetry is significant, finding the right bra size may become difficult. A breast reduction and a breast lift are in essence the same operation. In both operations the breasts are lifted whereby the nipple is placed in a higher position, but in a breast reduction breast tissue is also removed. During a breast lift there is the possibility of enlarging the breasts by the placing of breast prostheses.

During the first consultation the options for your breasts are judged and the options and consequences thereof will be discussed. A breast reduction will result in scars and may affect the sensitivity of the nipples and in some cases the sensation may be lost. The chances of being able to breastfeed after a reduction/lift will decrease significantly. This all depends on how much the breasts need to be lifted and how much breast tissue is to be removed. As with all procedures it is important that your weight and general health are acceptable.

The procedure takes place under general anesthetic and takes about 1½ to 2 hours. Because the areola will be raised and often made smaller, it will result in a scar around the areola. The skin in the lower part of the breast will need to be removed, which will result in a vertical scar from the areola and a horizontal scar under the breast. All together these scars will have the shape of an “anchor”. In a case of good elasticity of the skin and when the necessary reduction/lift is not too excessive, the horizontal scar under the breast can be avoided; the scar then takes the shape of a “lolly pop”. Most of the time, drains will be placed to drain any fluid and blood that may accumulate after the procedure. The procedure can take place as a day case and the removal of drains and follow up are done in the consulting room. The stitches are all dissolvable and in general do not have to be removed.

The drains are removed after 1–3 days. After removal, a sports bra without underwire needs to worn to protect the breasts and to allow the healing process to proceed undisturbed. It is important the bra is supportive and comfortable as it needs to be worn day and night for six weeks. In the case of a “lolly pop” technique being used, the skin under the breast will be will be wrinkled and swollen. This skin will smoothen out in 6–12 weeks. The final scar will depend mainly on your personal healing. All breasts are different with regards to firmness and fat tissue, the elasticity of the skin (stretch marks) and the healing process (scars). The total healing process will take a year, but after 3 months it will be more or less clear what the results will be. After a breast reduction/lift the aging process will continue and further changes can occur after changes in body weight, undergoing pregnancies and menopause. This too is different for all women. It is advisable to always wear a supportive bra during the day.

The exact cup size after the procedure is difficult to determine because of the difference among the various brands of bras. Most of the time it is desirable that the breasts are in proportion to the rest of the body. The nipples need to be in the right place and the breasts need to be about the same shape and size. Healing takes about a year in total but the result can be judged after 3 months when the wound is around 70 %. healed.

All operations run the risk of bleeding and infections. In general, these are to be treated well with little consequences for the final result. By moving the nipple upward, the sensitivity can change, decrease or disappear. In rare instances, the circulation of the areola-nipple complex is inadequate resulting in partial or complete necrosis of the areola/nipple. The chances of breastfeeding after a reduction/lift depend on the size of reduction and the amount of lift but in general are small. Some scars are less favorable and sometimes treatment or revision may be necessary.
